今日５月２６日我が家にも１人１０万円の特別定額給付金の 申請書が届きました。仕事が行政書士なので申請にはいつも 慣れているつもりでしたが、初めて高額（我が家にとっては）な国からの 支給金なので緊張しました。 僭越ながら、業務上の視点から申請にあたって注意点をお知らせします。 ①振込希望者の添付書類に銀行の通帳かカードの写しとありますが 必ず氏名のカタカナが載っているものを添付してください。 通帳は漢字記載しかない場合がありますので、通帳の裏面などを 確認して通帳表面と裏面の両方をコピーして添付してください。 キャッシュカードは比較的カタカナが記載されています。 ②おうちにコピー機がないお宅は免許証や通帳やカードをコンビニなどで印刷 されると思いますが、くれぐれも置き忘れにご注意ください。 ③１、申請書に<添付書類１>（左側真ん中辺） 申請者の「本人確認書類」 に必ず、貼付しました欄のレ点を記載してください。 ２、給付対象者の四角のチェック欄は受給を希望しない人が✖印をつけるため ですので、給付を希望する方は、ここは空欄にしておいてください。 ３、受取方法は、A：指定口座 B：窓口で直接受け取る に分かれているのでここもAかBの選択チェック欄にレ点を記入 してください。 右側真ん中辺にも<添付書類２>振込先金融機関口座確認書類に 貼付しました欄のレ点を記載してください。 ④念のため申請書と添付書類は、ご本人控えとしてコピーをお勧めします。 ⑤指定の封筒（ブルー）は切手は不要です。 封筒の中身は申請書と添付書類だけ入れてください。 封筒の裏面に差出人住所とお名前の記入もお忘れなく。 ⑥申請期間は、本日５月２６日（火）～８月２５日（火）の ３か月間のあいだです。 ご不明な点は同封の掛川市役所企画政策課 ０５３７－２１－１２１７ 新型コロナ緊急支援対策室までご連絡をお願いします。 |
